Welcome to the online clinic disaster preparedness questionnaire.
The questionnaire consists of seven sections, but you do not need to complete them all in one session. When you complete a section, your answers will be saved and your browser will go to the next section of the questionnaire. You can navigate to any section using the buttons in the upper left hand corner and save your progress at any time by clicking on the "Save Progress" button.
To assist you, your clinic’s answers from the prior questionnaire have already been filled in. If you would like to keep the same answer, click the "Keep" button. Otherwise, just change your answer. Green highlighting tells you which questions you have answered. You can change your answer at any time by clicking the "Change" button.
Please answer all questions as completely as possible. You may submit comments for any question by typing in the comments field.
A glossary of terms used in the questionnaire is available. You can access it at any point by clicking on the "Show Glossary" button.
For confidentiality, you will be asked to "log in" to the questionnaire by providing a password for your clinic. The first time you log in you will be asked to provide some information about your clinic and the person completing the questionnaire.
