In order to provide federal and California Indian Health agencies with state-wide surveillance on clinic disaster preparedness, a questionnaire was sent out and completed by all California Indian Health Clinics in the winter and late summer of 2006; an additional follow-up questionnaire is planned for summer 2007. An ongoing project is focusing on creating an online form for clinics to submit their responses via this website. The questionnaire and the executive summary of the results of the equestionnaire are available below.
The results from the Clinic Disaster Preparedness Questionnaire were used by the Indian Health Program to identify specific clinics who would receive on-site technical assistance from expert consultants. The Indian Health Program consultants traveled to each clinic and met with administrators, staff, and tribal representatives to aid in the development of an emergency operations plan for each clinic, based on the templates available on this website and at the semi-annual workshops.
Establishing Local Partnerships

In addition, the inter-agency collaboration seeks to encourage and aid in the development of effective partnerships between California Indian Health Clinics and local public health and emergency medical services agencies. To that effect, a sample of the joint letter from the Indian Health Services and Indian Health Program, as well as the following lists of Clinics, Public Health Departments and Officers, and Emergency Medical Service Agencies in California are listed below:
